Job Classification:  Direct Service Worker

 

Position Title:  HFI/RI Family Support worker

 

Job Status:   Regular, full-time, non-exempt, salaried

 

FTE: 1.0 FTE

 

Job Summary: The Family Support Worker is responsible for the provision of home visitation services, information on child development, and parent-child interaction to enrolled families, including high-risk and the emerging immigrant and refugee populations within Rock Island County. Responsible for assisting and supporting families in goal setting and developing their Individualized Family Support Plan.

 

Work Site:   Main office with travel necessary within Rock Island County.

Service delivery will occur in the homes of program participants within RI County.

 

Supervisor:  HFI Program Coordinator

 

 

Specific Duties:
  • Provide parenting support services to assigned families i.e. develop and carry out service plans with families enrolled in the program.

  • Provide services to families in person and/or by telephone, which may occur during normal business hours, evenings, or on weekends.

  • Conduct outreach and program participant recruitment with physicians and other health and human service organizations.

  • Assist families with information and referral linkages to community resources, as needed.

  • Participate in agency and staff development activities.

  • Facilitate parent groups, as needed.

  • Assist with program participant screening and assessments, as needed.

  • Maintain complete and accurate client case records.

  • Weekly data entry required into statewide database system and agency client tracking database.

  • Successfully complete mandatory HFI Assessment Track and Family Support Worker training

  • Attend and complete any other recommended HFI training(s)

  • Have knowledge and familiarity with Microsoft Office Programs

 

General Duties:
  •  Maintain personnel records (i.e. time sheet and mileage/expense reports)

  •  Attend scheduled CAC staff meetings and HFI team meetings

  •  Assist in general CAC planning, public activities, and fundraising as appropriate

 

Qualifications:

 

  • Bachelor’s Degree in Social Work, Child Development, Nursing, Education, or related field required.

  • Knowledge of health, social and environmental issues affecting the community, including diverse ethnic populations.

  • Knowledge of child growth and development and family dynamics.

  • Good written and verbal communication skills.

  • Must maintain confidentiality standards.

  • Must be willing to work some flexible hours, as needed.

  • Must be able to attend out of town, overnight training.

  • Must have a valid driver’s license, proof of insurance and provide own transportation

  • Must comply with local Medical Center/ Health Systems (Iowa Health Systems, Genesis Health Systems etc.) requirements for routine engagement of consumers within their systems/ facilities. This will include completion of a 5 panel urine drug screening and the receipt of/ maintaining as up-to-date (as warranted/ required) TB testing, MMR, Hep B, Varicella/ Chicken Pox, Influenza (annual renewal required) and TDap Immunizations/ vaccinations. Wavers may potentially be available pending the policies/ requirements/ regulations of the implementing Medical Center/ Health System. In instances of exception/ exclusion documentation/ waver must be provided/ completed substantiating same. Note: no wavers (pending medical indication) available/ accepted for the influenza and TB vaccination requirements. Exclusions from receipt of Influenza vaccination must be provided by the Employee’s Physician. 

  • Bi-lingual (English/French) (English/Spanish), a plus.
